บทคัดย่อ (THAI):

การศึกษาครั้งนี้เป็นการวิเคราะห์ต้นทุนและคุณภาพชีวิตของผู้ป่วยโรคไตวายเรื้อรังในจังหวัดเชียงใหม่ โดยมีวัตถุประสงค์เพื่อศึกษาต้นทุนในการดูแลรักษาผู้ป่วยโรคไตวายเรื้อรังในมุมมองของผู้รับบริการ และระดับคุณภาพชีวิตของผู้ป่วยโรคไตวายเรื้อรัง ที่ได้รับการบำบัดทดแทนไตด้วยการฟอกเลือดด้วยเครื่องไตเทียมและการล้างช่องท้องแบบถาวร ใช้การสัมภาษณ์ด้วยแบบสอบถาม ข้อมูลทั่วไปทางประชากร ต้นทุน และคุณภาพชีวิต กลุ่มตัวอย่าง คือผู้ป่วยโรคไตวายเรื้อรังจำนวน 280 คน แบ่งเป็นผู้ที่ฟอกเลือดด้วยเครื่องไตเทียม 104 คน และผู้ที่ฟอกไตทางช่องท้องแบบถาวร 176 คน
ผลการศึกษาพบว่าต้นทุนรวมในการดูแลรักษาผู้ป่วยโรคไตวายเรื้อรัง ที่ได้รับการบำบัดทดแทนไต มีมูลค่าทั้งหมด 13,149.49 บาทต่อปี เป็นต้นทุนชัดแจ้ง คือค่าเดินทางของผู้ป่วยและญาติ 5,836.96 บาทต่อปี และต้นทุนแอบแฝง ซึ่งได้แก่ รายได้ที่ต้องสูญเสียไปจากการป่วย เวลาที่ใช้ในการรอรับการตรวจรักษา และการเดินทางของผู้ป่วยและญาติ 7,312.04 บาทต่อปี ผู้ป่วยโรคไตวายเรื้อรังที่ได้รับการบำบัดทดแทนไตด้วยการฟอกเลือด มีต้นทุนรวม 15,572.68 บาทต่อปี ต้นทุนชัดแจ้ง 7,102.40 บาทต่อปี ต้นทุนแอบแฝง 8,470.28 บาทต่อปี ส่วนผู้ป่วยที่เข้ารับการบำบัดด้วยการล้างช่องท้องแบบถาวร มีต้นทุนรวม 11,717.61 บาทต่อปี มีต้นทุนชัดแจ้งเท่ากับ 5,089.20 บาทต่อปี และต้นทุนแอบแฝง 6,628.41 บาทต่อปี
ผลการศึกษาคุณภาพชีวิตของผู้ป่วยโรคไตวายเรื้อรังพบว่า มีค่าคุณภาพชีวิตเฉลี่ย 2.13 โดยที่ผู้ป่วยที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม มีค่าเฉลี่ย 2.07 ส่วนผู้ป่วยที่ได้รับการบำบัดด้วยการล้างช่องท้องแบบถาวร มีค่าเฉลี่ย 2.17 ซึ่งแสดงว่า การป่วยด้วยโรคไตวายเรื้อรัง มีผลรบกวนคุณภาพชีวิตในระดับปานกลาง ทั้งการฟอกเลือดด้วยเครื่องไตเทียม และการล้างช่องท้องแบบถาวร

This study is analyzed cost and quality of life of chronic renal failure patients in Chiang Mai. Objectives of this study are to investigate cost of chronic renal failure patients with kidney replacement therapy and to identify quality of life of chronic renal failure patients. Besides, questionnaire is implemented as a tool for compiling data include general population information, cost and quality of life. The sample group consists of 280 person, which selected from hemodialysis patients 104 person and continuous ambulatory peritoneal dialysis patients 176 person.
The finding indicated that total cost for take care of chronic renal failure patients was equivalent to 13,149.49 Baht per year. Which, explicit cost are costs of patient and relatives transportation equal to 5,836.96 Baht per year and total implicit cost equal to 7,312.04 Baht per year.
The hemodialysis patients have total cost equal to 15,572.68 Baht per year, include explicit cost equal to 7,102.40 Baht per year and implicit cost equal to 7,312.04 Baht per year. The continuous ambulatory peritoneal dialysis patients have total cost equal to 11,717.61 Baht per year, include explicit cost equal to 5,089.20 Baht per year and implicit cost equal to 6,628.41 Baht per year.
The finding indicated that average quality of life of chronic renal failure patients was equivalent to 2.13 point. The hemodialysis patients have average quality of life equal to 2.07 point. The continuous ambulatory peritoneal dialysis patients have average quality of life equal to 2.17 point. So the chronic renal failure have effect to quality of life of patients to average effect either the hemodialysis patients and the continuous ambulatory peritoneal dialysis patients.
