การศึกษานี้มีจุดมุ่งหมายเพื่อวิเคราะห์ต้นทุนและคุณภาพชีวิตของผู้ป่วยโรคไตวายเรื้อรังที่รักษาด้วยการฟอกเลือดและล้างไตผ่านช่องท้องโรงพยาบาลแพร่ จ. แพร่ ประจำปีงบประมาณ 2556 โดยแบ่งการศึกษาเป็นการศึกษาต้นทุนการรักษาผู้ป่วยไตวายเรื้อรังที่รักษาด้วยการฟอกเลือดและล้างไตผ่านช่องท้อง ในมุมมองของผู้ให้บริการ โดยใช้ข้อมูลทางการเงินจากหน่วยงานที่สนับสนุนการรักษาผู้ป่วยไตวายเรื้อรังที่รักษาด้วยการฟอกเลือดและล้างไตผ่านช่องท้อง และใช้วิธีคำนวณต้นทุนแบบสมการเส้นตรง
ผลการศึกษาพบว่า ต้นทุนรวมของการรักษาผู้ป่วยไตวายเรื้อรังด้วยการฟอกเลือด 12.54 ล้านบาท เป็นต้นทุนทางตรง 6.06 ล้านบาท และต้นทุนทางอ้อม 6.48 ล้านบาท และมีต้นทุนการให้บริการ 1,287.69 บาทต่อครั้ง ต้นทุนรวมของการรักษาผู้ป่วยไตวายเรื้อรังด้วยการล้างไตผ่านช่อง 2.95 ล้านบาท เป็นต้นทุนทางตรง 1.17 ล้านบาท และต้นทุนทางอ้อม 1.78 ล้านบาท และมีต้นทุนรวมของให้บริการ 1,128.64 บาทต่อครั้ง การศึกษาคุณภาพชีวิตของผู้ป่วยไตวายเรื้อรังที่รักษาด้วยการฟอกเลือดและล้างไตทางช่องท้อง เป็นข้อมูลปฐมภูมิที่ได้จากการเก็บแบบสอบถามผู้ป่วยไตวายเรื้อรังที่รักษาด้วยการฟอกเลือดจำนวน 52 คน และการรักษาด้วยการล้างไตทางช่องท้องจำนวน 151 คน ซึ่งประเมินด้วยแบบประเมินคุณภาพชีวิตทั่วไป EQ-5D และแบบประเมินคุณภาพชีวิตเฉพาะโรค ผลการศึกษาพบว่า คุณภาพชีวิตของผู้ป่วยไตวายเรื้อรังที่รักษาด้วยการฟอกเลือดอาการเจ็บป่วยที่เกิดจากโรคไตวายเรื้อรังไม่มีผลรบกวนผู้ป่วย และผลการศึกษาคุณภาพชีวิตของผู้ป่วยไตวายเรื้อรังที่รักษาด้วยการล้างไตผ่านช่องท้อง อาการเจ็บป่วยที่เกิดจากโรคไตวายเรื้อรังมีผลรบกวนต่อผู้ป่วยน้อย

The aim of this dependent research is to analyze cost and quality of life of Hemodialysis and Peritoneal Dialysis patients at Phrae Hospital in 2013 fiscal year. The study is divide into two parts. The first part is about the cost of treatment for both Hemodialysis and Peritoneal Dialysis patients in the view of service providers. Financial information from the department that support the treatment we collected and the linear equation was used to calculate the costs.The study found that the total costs of treatment by Hemodialysis method is 12.54 million baht which are spent into 6.06 million baht direct and 6.48 million baht indirect cost. The proportion of the direct costs as labor cost: cost of materials: investment costs is 76: 17: 7 and the service cost is 1,287.69 per time. While the total costs of Peritoneal Dialysis method is 2.95 million baht which are 1.17 million baht direct costs and 1.78 indirect costs. The proportion of the direct costs as labor cost: cost of materials: investment cost is 71 : 22 : 7 and the service cost is 1,128.64 baht per time. The research got the primary data for the second part of the study from gathering the EQ-5D and KDQOL-ST questionnaire which refer to the quality of life of CKD patients. The research collects data from 52 patients of Hemodialysis patients and 151 patients of Peritoneal Dialysis patients. The research found that there is no effect on quality of life in Hemodialysis patients while there is a negligible effect in Peritoneal Dialysis patients.
